Mandatory Pre-Participation Forms



All returning and incoming student-athletes at the University of Wisconsin-Whitewater must complete all forms listed prior to any participation in intercollegiate athletics. Do not send any printed forms in by mail or give to coaches.

All medical and eligibility forms will be completed online through individual student-athlete accounts on JumpForward. Information will be sent through the student-athlete's campus e-mail (@uww.edu) from JumpForward.

FRESHMEN/FIRST TIME TRANSFERS
Medical Forms

Physical Form:
This form must be printed and presented to your physician during your physical appointment. The physical must have been completed no later than six months prior to participation in any activities. The physical must be administered or supervised by a physician (NCAA Bylaw 17.1.5). This form will be mailed back to the address below or this form may be attached after completion and submitted online. See How to Attach a Document to JumpForward.

Medical History Questionnaire: This form must be completed and printed off prior to your physical. The physician completing your physical must also sign this form. This form will be mailed back to the address below or this form may be attached after completion and submitted online. See How to Attach a Document to JumpForward.

UW-Whitewater Insurance Form:
This form will be completed and submitted through JumpForward. It will ask for insurance information regarding the policy that you are covered under.

Copy of Insurance Card: A copy of your insurance card front and back must be submitted. The copy may be attached to your UW-Whitewater Insurance form. See How to Attach a Document to JumpForward.

NCAA Insurance Form:
This form will be completed and submitted through JumpForward. The NCAA requires every student-athlete to have insurance. This form acknowledges that you have an insurance policy that meets the requirements of the NCAA.

ADHD: This form only needs to be completed if you are diagnosed with ADHD and are currently taking prescription medications. Please print, fill out and have your physician complete the form.

Documentation of Sickle Cell Status: The NCAA requires all student-athletes provide documentation of their sickle cell status. Most infants are tested at birth.

If you were born in either the State of Wisconsin or State of Illinois, a release of records was sent via JumpForward. Please submit that information through JumpForward. The release is due by July 25, 2019.

If you were born outside of Illinois or Wisconsin, you have multiple ways to satisfy the requirement.
  1. Contact your physician to see if they have a copy of your records
  2. Have a blood draw conducted by your physician to determine your sickle cell trait status
  3. Have a blood draw conducted by the UW-Whitewater Health and Counseling Services. Directions for this were included on the Sickle Cell information sheet.

All forms, excluding the Sickle Cell Release forms, are due by August 1, 2019.

RETURNING STUDENT-ATHLETES
Medical Forms

Your returning medical and eligibility forms have been sent to your JumpForward account. As a returning student-athlete, you will need to complete the NCAA Insurance Form and the UW-Whitewater Insurance Form. Additionally, a parent/guardian letter and ADHD document were sent. The parent/guardian letter does not need to be submitted back. The ADHD form only needs to be turned in if you have ADHD and are taking prescribed medication. The NCAA insurance and UW-Whitewater insurance forms will be due by May 14, 2019.

You are still required to submit a copy of both sides of your current insurance card. Please attach the insurance card to your UW-Whitewater insurance form. See How to Attach a Document to JumpForward.

On approximately August 1, you will receive a health history questionnaire that will be completed online. All of these forms must be completed prior to your athletic participation.
